With questionnaires, you can use a form to collect info from a new hire or another team member, as part of an onboarding route or as a standalone task. When completed, you can find the answered questions on the Questionnaires tab of a new hire’s profile, or in the completed task on the Task List.
You create each questionnaire template once, using the following response types:
Paragraph, for a long-form text response
Dropdown List, to select one item from a list of options
Multiple Choice, to select one or more items from a list of options
Checkbox, for a single true or false response
Date, to select a specific date
How to create questionnaires
Admins can set up questionnaires in their company settings.
Click the gear icon ⚙️ in the left sidebar.
Click Questionnaires.
Click + Questionnaire Template.
Enter a name for the template.
Click +Add Question.
Enter a title and description for the question.
Choose an option from the Response Type dropdown.
Create options as needed for Dropdown and Multiple Choice response types.
Decide whether to make the question required.
Click Save Changes.
Continue adding questions.
Click Save Changes.
